Highly Reliable Hand-Offs: The SLCH Heart Center Experience

In 2014, the Heart Center at SLCH embarked on a project to create a highly reliable safe hand-off process for patients returning to the ICU following open heart surgery. This presentation shares the details of the process, and the work it stimulated to sustain change, spread this improvement to other hand-offs within the Heart Center, and scale the process beyond St. Louis as part of a national quality improvement collaborative.
